How to Fill It Out

  1. Find all passports from your time in Cyprus, including expired ones
  2. Write down every departure and return from the stamps: dates, country, passport number, and stamp page
  3. If some stamps are missing, verify with flight tickets, boarding passes, or card statements
  4. List all residence addresses in Cyprus with dates

Days are counted the same way as for tax residency: the arrival day in Cyprus is a day of presence, the departure day is a day of absence. The final table must match your passport stamps.
